The Role of the dopamine D3 receptor in Cognition and Disability in Schizophrenia

Cognitive deficits and impairments in everyday functional skills are among the major contributors to the impairments in everyday functioning commonly seen in people with schizophrenia. In the recent past, there have been a number of cognitive remediation strategies developed for use in schizophrenia, with some of these interventions having substantially greater benefits than older efforts. This lecture will review the state of the art of treatment of cognitive and functional deficits in schizophrenia. As part of this task, it is important to consider what types of outcome measures are best suited to detect changes in cognition and functioning. On the treatment side, there are several different questions to address. These include identifying the treatment strategies that work the best, at what level their benefits occur (cognitive performance, functional capacity, brain activation, triggering positive responses in neuroplasticity, improved real-world functioning), and what types of concurrent interventions are required to lead to real world functional gains. One promising mechanism receiving recent attention is dopamine D3 receptor antagonism. There are several newly developed medications that target this receptor and the basic science and clinical data suggesting cognitive benefits will be reviewed. Several recent other pharmacological and remediation treatment successes will be reviewed as well, with their implications for the current best strategies to reduce disability in people with schizophrenia.
